CIVIC CENTER (FACILITY USE) APPLICATION AND PERMIT INSTRUCTIONS
ü All applications must be submitted no later than THREE WEEKS in advance of first date of use. Late applications shall be assessed a late fee of $30.00
ü Please submit the $20 application fee for facility use ($10 application fee for hanging a banner on the fence line) and the “Application and Permit for Civic Center Use of School Site Property” and the certificate of insurance to the Business Services office. The application and permit will be returned to you if the certificate is not attached. Application fees are non-refundable.
ü The Business Services office will notify you of the total amount of fees due after your application has been processed. All fees are due prior to permit approval. Upon receipt of all fees, certificate of liability insurance and permit approval you will be furnished with a copy of the approved application/permit.
ü You are required to provide a certificate of liability insurance with minimum limits of $1,000,000. In addition, the Lompoc Unified School District is to be named additional insured. Certificate holder should read:

ü The school site must be listed on the application. Verify that all dates of requested use arelisted. If necessary attach a sheet with the dates of use to your civic center application.
ü Please give a complete description of your intended use of the facility. If this is a youth activity, please provide the age group of the participants.
ü One of the boxes for “admission charge” must be checked. If admission is charged, be sure to fill in the amount, and describe how the proceeds will be used.
ü Please complete and submit ALL four pages of the Civic Center Application with application fee three weeks in advance of requested date of use. Incomplete applications will not be processed.
ü Banners may be hung for no longer than 30 days in a 60 day period.
