Our school nurses and health office staff are here to keep your student healthy, safe and ready to learn. Find screenings, forms and guidelines below. 

 

Credentialed School Nurses

LUSD employs licensed Registered Nurses who hold School Nurse credentials issued by the Commission on Teacher Credentialing (CTC). Our nurses are bachelor, but mostly master-prepared professionals. We do have one doctoral prepared nurse on our team. These nurses have extensive experience across diverse nursing specialties. LUSD school nurses play a vital role in promoting student health, safety, and overall well-being.

Credentialed school nurses use the nursing process, clinical expertise, and professional judgment to design and implement health services programs that ensure that all students will receive the optimum benefit from their educational experience. This is accomplished by working collaboratively with students, families, teachers, other members of the multidisciplinary team, and utilizing resources within the community, to meet the physical, mental, emotional and social health needs of students. School Health Office Staff

LUSD employs licensed vocational nurses and health clerks who work under the direction of the assigned administrator and the professional direction of a licensed credentialed school nurse. These school health office staff members perform specialized physical health care services for pupils with disabilities and chronic health problems. They also provides basic first aid to students who present to the health office. More information is available at schools

 

Health Screenings

LUSD conducts required health screenings at key grade levels throughout the year. Parents and guardians are notified if their child does not pass a screening.

 

  • Hearing Screening

    LUSD screens students for hearing in T-K, K, 2,5,8, and 11. Parents are notified if their child fails the hearing screening test. Hearing screenings may be waived upon presentation of an appropriate certificate from a physician or audiologist.
  • Vision Screening

    LUSD screens students for vision, including tests for visual acuity, in T-K, K,2,5,8 and 11. Students are screened upon first enrollment and at least every third year thereafter. Parents are notified if their child fails the vision screening test. Visual screenings may be waived upon presentation of an appropriate certificate from a physician or optometrist.
  • Color Vision Screening

    LUSD screens all male students in first grade for color vision. Color vision deficiency, also called Color Blindness affects 8% of boys and 0.5% of girls, or about 1 out of 25 children. The affected child sees colors differently than most of his or her classmates. Screenings alert parents and teachers that there may be a color perception issue and a follow-up evaluation from an eye care professional.

Download the forms you need below. Forms are available in both English and Spanish. If you have questions about which form to use, contact your school's health office.

  • ​​​​​​​Students who need to take medication during the school day can be assisted by school health staff. This includes prescription medications as well as over-the-counter remedies, nutritional supplements, and herbal remedies.

    To have medication administered at school, families must follow the steps below.

    1. A California-licensed health care provider must complete and sign a District Medication Authorization form. A separate form is required for each medication.
    2. The parent or guardian completes the authorization section on the same form, giving permission for their child to receive the medication at school.
    3. A parent, guardian, or adult representative delivers the medication to school in its original pharmacy container, labeled with the student's name. Over-the-counter medications must be in the original, unopened package.
    4. A new authorization form is required at the start of each school year and any time a prescription changes.

    A note on field trips and school activities: Medication administered during field trips or other school-related activities requires a written statement from the student's authorized health care provider and a separate written statement from the parent or guardian.
